Not so much an error as a continuation of the New Teen Titans madness 
Titans Vol. 4 essentially stops with #32 and becomes New Titans Vol. 2 (unless i am mistaken) with #33 and onwards, in pure Titans fashion.
That means that you should probably split issues #33 onwards in a new series rather than keep them in Titans Vol. 4 and also change the New Titans series from 1988 to New Titans Vol. 1 (the one that continued from New Teen Titans Vol. 2
).
For the love of God, i don’t know why DC keeps doing this to my boys & girls.
edit : i just read another post where you explain that you like to keep titles together as much as possible, so apologies if i am treading the same ground here, but i just read it after posting. Don’t want to make you explain the same things over and over 
Hi, we were aware of this but can’t do anything until a series has ended/concluded. We are keeping this series as is. Changing a series halfway trough releasing messes up peoples database.
We’re trying to set new standard as there weren’t set rules for this before, by trying to keep a series together as intended as much as possible. When a title change is similar to X-Men > Uncanny X-Men we are keeping them together. If there is a clear difference (Incredible Hulk > Incredible Hercules) we’re splitting them up. Creating a new volume just because a title changes messes up a lot in the future and we’re just trying to prevent that now. A new volume should always start with a #1 unless the title change has a clear difference. We are looking into a solution for this though.
